athletes as well as other sports celebrities and Awards for tournament and contests winners.

This action creates an electronic signature that has the same legal force and effect as a handwritten signature.

Included within this consent are the following terms and conditions:

- The Achievements Unlimited Basketball School has liability insurance policies. However, if the camper has any specific medical problem, please notify us in writing and submit with application. A qualified trainer will treat minor injuries. Serious injuries or illnesses will be treated at the closest hospital emergency room.

- I hereby state that the Achievements Unlimited Basketball School is not responsible for any previous injury or recurrence of any injury of my child prior to the first day he/she registers or occurring outside of camp operating hours and daily activities. The basketball school will assume responsibility only for injuries incurred while my child is participating in camp activities supervised during the enrollment period.

- I hereby agree to assume complete financial responsibility for any personal injury or property damage incurred as a result of a willful or negligent act of my child while he/she is a camper at the Achievements Unlimited Basketball School or on the ground of Greensboro Sportsplex, Ardrey Kell High School, Community Middle School, Morrison YMCA.

- I hereby agree that in the event my child is unruly and the appropriate officials of the Achievements Unlimited Basketball School are of the opinion that such child may harm himself or others, or damage property, they can terminate such child as a camper and I agree to arrange for such child’s immediate transportation home, without refund of fee.

- All events are subject to documentation by the Achievements Unlimited production crew and any other media outlets. Achievements Unlimited, its affiliates, or sponsors reserve the right but not the obligation to photograph, file, and capture participants and participants’ likeness, voice and other actions and effects. I further agree that these images may be used in the production, distribution, promotion, publicity and advertising for Achievements Unlimited and other future events for perpetuity.
